LICENSE FOR TRANSPORTATION OF DANGEROUS GOODS

Contents of the License for Transportation of Dangerous Goods:

a) Name, address, and contact telephone number of the licensed entity full name and title of the legal representative.

b) Type and classification group of dangerous goods.

c) Route and schedule of transportation.

d) Validity period of the license.

For licenses issued per shipment, additional information must be provided, including vehicle details and driver/operator information.

• The license template and hazardous goods warning labels are issued and regulated by the competent authority.

• Licenses issued per shipment or per period (as requested by the transport enterprise) shall be valid for no more than 24 months and shall not exceed the vehicle’s inspection or operational lifespan.

Dossier for Initial Issuance or Reissuance of Dangerous Goods Transport License:

1. Dossier for Transporting Dangerous Goods of Class 5 and Class 8 Includes:

a) Application form for transport license as prescribed in Appendix IV of the relevant Decree.

b) A copy of the Road Transport Business License specifying freight transport (for road operators), or a copy of the Enterprise or Cooperative Registration Certificate (for inland waterway operators).

c) Original or certified copy of the Vehicle List accompanied by copies of valid safety and environmental inspection certificates for each vehicle (applicable to per-shipment cases).

d) Original or certified copy of the Driver List for those operating dangerous goods vehicles. For inland waterway transport, include a copy of the Specialized Certificate (required for per-shipment transport).

e) Original or certified Transport Plan, specifying the route, schedule, and emergency response measures for chemical incidents during transit.

f) Certified copies or original Certificates of Conformity to standards and regulations, or test results and inspection certificates for dangerous goods packaging, containers, and materials—complying with regulations of the Ministry of Industry and Trade and applicable technical standards for motor, rail, or inland waterway transport.

2. Dossier for Transporting Dangerous Goods of Class 1, 2, 3, 4, and 9 Includes:

a) Application form as per Appendix IV of the Decree.

b) A copy of the Road Transport Business License (with freight transport scope), or Enterprise/Cooperative Registration Certificate (for waterway operators).

c) Original or certified copy of the Vehicle List and valid Safety and Environmental Inspection Certificates (applicable to per-shipment).

d) Original or certified Driver List. For waterway transport: include Specialized Certificates.

e) Original or certified Transport Plan detailing routes, schedules, and emergency measures in case of fire or explosion.

f) Original or copy of the Industrial Explosives Supply or Sale Contract, or permit for trial explosion, or decision for disposal of explosives from competent authority.

g) Original or copy of Inspection Minutes issued by competent Fire and Rescue Police, verifying safety conditions for the escort personnel, vehicle operators, and transport vehicles (attach original for comparison).

h) Copy of the registration detailing the type, quantity, and receiving schedule from the competent authority managing the explosives warehouse, or written approval from the provincial People’s Committee for loading/unloading locations.

i) Copy of the License for Use of Industrial Explosives or written permission for import issued by a competent authority.

k) Copy of written permission issued by a competent authority for export or international transport of industrial explosives (for outbound shipments).
