- Power of attorney
- Official letter for accounting method and invoice usage registration
- Fixed asset depreciation registration form (if applicable)
- Form 06/GTGT for tax calculation method registration (attached with a notarized office lease contract to apply for credit method)
- Bank account registration appendix (submitted to the Department of Planning and Investment)
